Sri Lanka Customs explicitly requires this document to be completed and signed by the Sender/Consignor for the processing and final release of the goods upon arrival in Sri Lanka.

Failure to return the completed and signed form may lead to significant delays, potential penalties, and storage/demurrage charges, as the cargo cannot be legally cleared by Sri Lanka Customs without this declaration.
